I don't know what grade level you teach, but for my fifth graders this form works great! It includes mood (feelings), figurative language, rhyme scheme. In my opinion, this is appropriate for the grade levels which were indicated. It can certainly be added to if necessary, but I find it to be totally fine for my students. I don't quite know what you were looking for, but I would love to know.

TEACHING EXPERIENCE
Subject Area:
I have taught for 36 years at the 3rd, 5th, and 6th grade levels. Fifth grade is my first love. I teach all major subject areas. In addition to my regular classroom duties, I have taught in a Basic Skills summer school program for many years as well as taught workshops to co-workers in whole language and technology. My other duties at school include being "Teacher in Charge" and Student Council advisor.
