Beginning of the Year, Back to School - Student's Favorite Things

This is something I send home as homework the first week of school. I have the kids fill it out telling me what their favorite things are, so that I can learn more about them and have it on paper to refer to throughout the year. I file it in their student files so that I can spark conversations with them on different things and see what they have in common with me and their fellow classmates. I have also modified it so that it can be used for any grade level (see that #16 on the form can be filled in by you)
