This is a set of 5 behavior forms. These forms can be used when students are being disruptive in class. Instead of interrupting the whole class, have the students go to an assigned area where they will have to fill out one of these forms. It will take them five minutes or longer to fill out the forms, which will help them to think about their behavior and come back to whole class with a better attitude.

The five forms are:
1. Behavior Incident Sheet. In this form students have to answer a series of questions about how they broke the class rules, why they did it, and what they are going to do differently next time.

2. Talking Form. This form addresses talking and how it disrupts the class. Students have to copy the whole form before they can come back to class.

3. Respect Form. This form addresses how and why students should be respectful in the classroom. Students have to copy the form and then identify what they did wrong.

4. Sentences Form. This form has students write sentences. The sentence is on respect and responsibility.

5. Time Out form. This form addresses how getting an education is a privilege and how their behavior affects the teacher, the class and their selves. Students have to copy the form before they can come back to class. This is a great form to send out with students to detention or give to a student who is in a time out area.

TEACHING EXPERIENCE

As long as I can remember I have been a creative person. Beginning early on I was always involved in arts and crafts projects at home and in school. At the age of 13 I began more formal training taking varying classes in illustration, commercial art and crafts. At the age of 16 I began on the job training in commercial art. I continued my education at California State University Long Beach receiving a Bachelors Degree in Art. After receiving my Art degree I went to work for an Advertising agency. I began as a graphic designer and worked my way up to an Art Director. After working in an advertising agency for more than 15 years I decided I wanted to do something more worthwhile. I had a teacher who really a positive role model in high school. I began volunteering in her graphic design class working with the students on projects. After volunteering for two years I decided I wanted to go back to school to get my teaching credential. After I received my , I began teaching in middle school. I have taught 6th, 7th and 8th grade Language Arts, Social Studies and Read 180. I have also taught 7th and 8th grade art. After teaching at middle school I moved on to teaching art at the high school level. I teach all levels of art including Art 1, Art 2, Art 3, Art 4, AP, Computer Graphics and Yearbook. I hope to be a good influence and inspiration to my students.
