This form (one for 10 point quizzes and one for 12 point quizzes) is designed to help you break down the data and guide your instruction. There is space to write the standard you are testing, as well as an area to create small groups for remediation or enrichment based on the data.
For more specific information on how to use this form, please visit my blog at http://teachinginroom6.blogspot.com/2012/01/breaking-down-quizzes.html
I LOVE this! This is going to make my grading and record-keeping so much easier! I am changing grade levels this year, and this is a great way to start off in an organized fashion, instead of having to rework all of my previous grade level documentation. THANK YOU!!

Simple and perfect! I have been doing this analysis by painfully writing notes for each child.... This concise sheet will save so much time while giving me the same valuable information! Thank you!
I saw your posting on the Sellers Forum page of Teachers Pay Teachers; so, I thought I would look at your freebie. I rated it for you.
I have a few suggestions that might help you to better market your products. First, create a cover page which can then be used as an attractive thumbnail to entice buyers. Add a second page which is a directions or suggestions page. List several ways you use this product in your classroom. This would be especiall helpful for your free item. Never assume that a buyer knows how, why, or when to use your product. Use the footer to put the copyright symbol, your name and date of creation on EVERY page. Items, especially free ones, have been "stolen" from TPT and posted on other sites to be sold. You have to protect all of your creations.
If you have time, I’d appreciate your input on my freebie listed below.
Thank you so much for taking the time to rate my product, as well as for the suggestions! All of this is so new to me :) I just posted the things from my blog that I had on GoogleDocs, here...so I figured everyone who was looking at them was coming from my blog. Great idea to have an explanation. Off to do that now!
I am now following you and have rated your freebie!
