Keep track of your classroom books as students use them by using this handy sign-out sheet. Make duplicate copies of this sign-out sheet, attach them to a clipboard, and you're good to go for the year. A suggestion would be to allow students to use a date stamp for "date out" and "date in". This will make it seem more like the school library. This packet also includes "This Book Belongs To" label and date card to attach to each classroom book. These can of course be duplicated.
