The Cold Probe Data sheet is a handy way to break down larger IEP goals and objectives into smaller, easier to document chunks. It is often used in Applied Behavior Analysis (ABA) programs designed for students with Autism; however I find it helpful with all of my students in a variety of goals from social to cognitive to fine/gross motor. By listing short term objectives or small steps to reach an objective/goal, it gives you an easy to read summary of their progress on that skill. Each box measures a skill across 8 sessions or attempts. Generally a skill is considered mastered once the child is independent across 3 attempts and then the skill is retested several days later to test mastery.
