This is a short power point presentation with a 3 page Course Questionnaire Evaluation form. This Course Questionnaire Evaluation was developed for NEW teacher use. It has been one of the most effective tools I have used to obtain student feedback, develop more effective and appropriate instruction. The form is written in small caps. Please do not purchase this form, if you do not like small caps. It looks fine and does the job. Teachers are recommended to add, deduct and modify as needed. This form provides excellent evidence as to what is going on in one’s classroom. On the form, the student name identification is optional. This should advance authentic answers. The purpose of the Course Questionnaire Evaluation is for the teacher to obtain student feedback; self evaluate, and modify instruction accordingly.
