I created this behavior progress report to help students see their behavior throughout the course of the day and the week. This chart can be used in a regular education class or a behavior class. At the start of the week the student staples the chart in to their agenda. Throughout the day the teacher and student decided if their behavior is acceptable and if so, the child draws a star in the appropriate box. Every day, the child takes the progress report home so that the parent is able to see what kind of a day their child had in school. The parents is then required to sign the chart and talk with their child. I have found much success with this chart and it has greatly improved my classroom atmosphere! Laura Ruscetta
This was a nice simple form, but if a student were to not earn a star during a specific class/lesson, there wasn't much space to explain what the infraction was. It's more time consuming to write the infraction in the comments area; I'd rather circle or check a box.
