I created this form for a student on the Autism Spectrum Disorder. This form was a wonderful tool between his parents and myself. I have kept it in a word document so that you can modify it as needed for your individual situation.
I found it really helped me know what to expect each day and just a took a couple moments at the end of the day to relay a lot of information that would have taken me a long time to write in daily "notes".
I taught EC from 1999 - 2011. I taught inclusion, resource and self-contained for a classroom of students with severe communication disorders and autism. I moved to a regular education clssroom this year teaching Kindergarten.
