Use this form any time you want to hold students accountable for small group discussions. There's a place for the date, recorder's name, the names of all of the group members, and a place to rate their level of participation. Then, the form asks what was discussed and if there were any disagreements. It's pretty simple and can be demonstrated to children (i.e., how to fill it out) in under five minutes.
