Looking for a way to keep track of independent reading? I created this system when I taught sixth grade in middle school. It's fairly simple. Students read for ten minutes in class each day, complete a weekly assignment, and must read at least one book each month. Everything you need to get started is included, with an overview of the materials, an assignment sheet, a weekly menu, and forms for students to complete at the beginning and end of the month. Best of all, it's simple and doesn't require much grading, so you can spend your time matching books to readers and talking with kids about what they are reading.
**2010 Update: I re-posted this as a PDF to alleviate some problems with font conversion. Otherwise, everything is the same.

I'm the author of two books for Heinemann: Summarizing, Paraphrasing, and Retelling, and The Forest AND the Trees: Helping Readings Identify Important Details in Texts and Tests. Both books help teachers to figure out and meet the needs of their students.
