This is a great contact form I use with my Spanish classes every year. Students fill the front of the page with their information on the first day of school. You may choose to have them take it home and bring it back the next day.
It's easy to keep track of parent contact because you can put all these forms in a binder and write in the back each time you contact a parent.

Karen: I have taught High School Spanish for 3 1/2 years in New York State public schools, 5 months of High School English Language Arts in Tennessee. And one year of Spanish from Pre-K through 8th grade in a private school (2011-2012). The one thing that has been consistent in my life is my volunteer work with the Pathfinders Club. For the past 14 years, I have worked with kids, staff (teen & adult) teaching them arts and crafts, home skills, survival skill, Biblical principals and so much more. My greatest joy is to now work with one of my students side by side and be able to see first hand how my time, patience and love has impacted her life.
