I use this form on all my field trips. It's a great management tool plus it provides me valuable feedback regarding the value of a trip from a student's perspective. We go over the form before each trip, and then again once we're back at school. You may be wondering about the teacher comment part being a time constraint. I use rubber stamps that say "Great Job" etc. for most kids, and hand-write responses for those who need to show improvement. It really makes students think about their actions.
