This is a generic book review form set up for students who need more structure and guidance when doing book reviews. It is very helpful at all levels, especially English language learners in ELD classes.
I have used these forms successfully at 6th, 7th, and 8th grade with ELD, regular, and advanced English students. Advanced English students were given this format as their first Book Review. It helped them understand what I expected in future, less structured book reviews.
You can have students do these book reviews with books you assign or as an activity that can go with their reading logs.
You have two pages which reinforce summary, response to literature, parenthetical citation, and almost every level of Bloom's Taxonomy. Students are also asked to calculate average number of words per book, so we can see how close they are getting to the California Standards requirement of one million words per year for 7th and 8th graders.

Help
I have taught every grade level from first through eighth in my twenty years of teaching. At both the elementary and middle school levels, I have worked as a Curriculum Coach and mentor teacher. Adolescent Skills, Study Skills, Journalism, ELD, English, and Advanced English are some of the courses I have taught. I have been teaching and continue to teach English, Advanced English, and ELD at the middle school level since the year 2000. I am presently teaching 7th grade only.
