This is a form I designed for my Reader's Workshop Notebook. It helps me to stay organized and allows me to jot down and useful, informal observations while my students meet with me for guided reading. I copy the pages back to back and put them in a binder for each group. Then I have the lesson plan and notes organized for future reference. I hope you find it useful.

It's a MAC file so I can't send it to you in word. I'd be happy to download it to google docs but I'm confused, in google docs can you edit a file? If you can, that would be news to me and I'd be happy to do it for you. :)

Sure. Question for you though: If I send it in GoogleDocs won't it still be a PDF and unable to edit? It's a Mac file so Word will not open it. Let me know and I'd be happy to send it.
