I made these pages to help me plan and record my observations during guided reading. I make copies and put them in a binder, using dividers for each group. Before my lesson I record my focus and plan my lesson. During each guided reading lesson I record my notes and observations for each student (how's their fluency, comprehension, improvements, needs, etc.). This record helps me plan future lesson and give appropriate feedback to students and parents.
