This is a great Independent Reading log I use with my students. Basically, the cover page includes 15+ possible ways to respond to independent reading on a daily basis as well as a rubric for evaluation. The log itself should be printed on both sides of a page, and includes space for their responses and teacher comments.
It's a good way to give students some say in how they choose to discuss the book they are reading on their own.
