Since going electronic, I have always done a 5-page lesson plan (one page for each day) which allows plenty of space for all the things my district requires: standards, student friendly objective, resource, procedure, assessment, etc. But I really like this layout because you can see the whole week at a glance. I will play around with it and tweak it to fit my school needs.
I do wish that your sample plan (the one where you have filled in the boxes) would have also been part of the download. I would have loved to see how you used your template to suit your needs. Mousing over the thumbnail just didn't enlarge it enough to see all the detail.

It's probably because it's editable and in a Word document instead of a PDF. I think the font is CK Handprint (free download online).

Sorry, it's made to be editable, so it's not saved as a PDF. I think it's a .docx because it was created on Word 2010 or newer???
