My listening center response forms are a great way for your students to respond to a listening center story!
You can model completing the listening center forms using this PDF on your interactive white board or by using a hard copy of the forms.
The 4 forms are designed to be used through a school year (each form is a bit more challenging). You could also use the forms to differentiate or to give the students choices. Use them in the way that best suits YOUR class!
Form A: Draw and Label (1 page form)
The students will record the book’s title and draw an illustration of a story event. They will label at least 8 things in the illustration.
Form B: Draw and Label, Write a Sentence (1 page form)
The students will record the book’s title, draw an illustration of a story event, label at least 8 things in the illustration and write one sentence about the event. (I switch to this form after my students have had some lessons about writing a good sentence.)
Form C: Indicate Opinion, Draw Event, Write Sentence, Write a Cloze Letter (2 page form)
The students will record the book’s title, their personal opinion of the book, draw the beginning event & write a beginning event sentence. They will complete a cloze letter (tell book title, make a recommendation, make a connection and tell the ending event).
Form D = Opinion, Characters, Settings, Events (2 page form)
The students will record the book’s title and their personal opinion of the book. They will write to identify the character(s), the setting(s), and 4 main events. Finally, they will draw the ending event.
***NOTE: This product has been updated March 2013. I have included the original forms as well as the new updated forms. Both sets contain the same components. Use the version you like best!

HI Jane,
I think I can help. First, make sure you drag the file out of your download file. Then, when you open it up, make sure you are opening it with Adobe Reader and not Preview (sometimes on a Mac, Preview is the default reader). Once you have it opened with Adobe Reader, you can "save a copy". You will get a pop up saying you cannot make any changes. Just click OK and it will save you a duplicate of the file. You could also go to "my purchases" and download another copy if you need it (again, make sure you open it with Adobe Reader). You should not need a password. If this doesn't work, just let me know and we'll get it sorted out.
