This is a homework log sheet that my students use when they come to class with an incomplete homework assignment. It puts all the responsibility of the "missing" homework on the student. It stays in my classroom (in the student portfolios) and when students come without their homework completed, they fill it in for the assignment they are missing while I collect it from the students who are prepared. When they eventually complete the assignments listed on their log sheet, they check it off in the space provided. Students can access it at any time to see if they are missing anything, and they do it without involving the teacher! Priceless. :)
It is also good to have this record (in the student's handwriting) if a parent ever needs to see the record of incomplete work.
What a terrific idea....makes the kids responsible for finishing missing homework, and provides a document to show parents in recurring cases.

What a great way for students to take responsibility for themselves. It is my hope that this form will cut down on the time consuming excuses that my third graders always seem to have. Great for parent conferences, too.

I like this sheet. It will help me keep track of the number of times homework is turned in late and will be useful when conferencing with parents. I agree that I graphic would have been nice, but it is not necessary. Thanks for sharing!
Oh... one idea might be a column for a parent signature.

I currently teach Pre-Algebra, Algebra, and Geometry to 7th and 8th graders in the Northern suburbs of Chicago.
