When I grew tired of using class time to write notes in student's planners about missing homework assignments, I created this slip that my students were required to fill out and glue in their planners if they were missing homework. I like to use this method because it puts the responsibility for the missing homework on the student where it belongs. On the slip students fill out the name of the missing assignment, the reason it was not turned in and when they will complete it (at recess, lunch or that night as extra homework). My parents liked the slips because when they are printed on colored paper, they really stand out in the agenda. Each page contains 6 slips.
