This Monthly Reading Logs PDF document is great for grades 1 & 2. I used these logs in my 2nd grade classroom last year and it made reading homework a breeze for both students and their parents. I would send home each month's log home at the beginning of the month, and then the completed log would be due at the end of the month. All 12 months are included.
