This survey is an excellent idea. I also get parents to email me with their contact address etc., so I can keep them up-to-date with their childs learning if THEY want to!
Excellent questions too! :) I would adjust some of them so they are open ended (parents more likely to respond) or on a 5 point scale to make it easier for you to collate the data somewhere!
