In an effort to teach my students how to self-monitor their learning, I created a combination checklist and rubric for the personal narrative unit of study. I think this is a user-friendly document for teachers and students since it allows the student to self-assess, allows the teacher to assess the child, and gives the teacher some space for feedback.

Rubric is hard to read and uses a poor choice of font.
I think if you're having the students make a value judgment, saying "Circle the number that ACCURATELY reflects how you think you did in this area" doesn't work without a WHY. I was thinking of using this as a peer review rubric rather than letting the teacher full out so much.
I do like some of the headings.

I'm a fourth grade teacher in Rhode Island. Before moving to Rhode Island, I taught fifth grade for three years in the NYC Public School System.
