This PowerPoint expalins what plagiarism is using Kaavya Viswanathan as an example. Then it goes on to show students how to avoid plagiarizing when completing reports.
There are 14 slides with the last one being a copy of the form that I use to hold students (and parents) accountable for the information on the PowerPoint. I use this every year with my sixth grade students and it seems to help cut down on the plagiarism cases.

I recently purchased your Plagiarism PowerPoint, and it is perfect for the lesson we would like to do school-wide with our students in the fall. I just wanted to check with you about permission to use your work. I don't see any copyright information on the product description page, but I'd rather check and be certain before I plagiarize the plagiarism lesson. :) If we can use it, how would you like us to give credit to you in the presentation, as your name is not currently in it?

Hello! Thanks for asking! Yes, you may definitely use the PowerPoint schoolwide. I have actually added a citation slide to the PowerPoint as a result of your question and re-uploaded the PowerPoint. It is a great addition as this will show how to give credit where credit is due. Thanks for helping me to make it better! :)

I am in my 18th year of teaching. I taught for ten years in the Orange County (Florida) Public School system at primarily the sixth grade level. During that time I taught Language Arts, remedial reading, social studies and exceptional student education (aka special education). Then for 6 years I taught sixth grade Math, Language Arts and Social Studies to sixth graders in a private, independent school. Now I am back in public school and am teaching students with Special Needs.
