This form allows you to keep track of 30 children and 8 different centers for an entire week. The index M=Monday, T=Tuesday, W=Wednesday, Th=Thursday, and F=Friday is used to mark which center a child goes in on a daily basis. This is especially important if you have small centers like Sand/Water which usually can only hold 2 to 3 children. With this form, you can keep track of which children have not had a chance to go to the center and those who have already gone. It also allows you to see which centers your children need to be rotating too, especially if they are only going to 1 or 2 favorite centers.
