This reading log and opinion recording sheet helps my students keep track of books they read during literacy center time.
After reading a book, students will record the title and color a small icon to show their opinion (loved it, liked it or it was O.K.).
This is a 2 page form that can be copied 2 sided. Students can record 10 books. Print and copy the forms and place them in a literacy center. When my students complete a form, they start a new one. We use them all year long!
A teacher's notes page is included.

I added third version of this recording form that includes a line for students to write a sentence telling an important event from the story.
This third set could be used when your students need a more challenging response form OR use the different versions to differentiate in your classroom.
