This form is for a reading log for nightly reading. It includes a space for parents to initial, the date, book title and author, if the story was fiction or nonfiction, as well as if the student liked the reading.
Also included is a sample "cover" for you to use. I created my own reading logs for my 2nd grade class using this file and some construction paper. Then I glued the cover on. This document has worked wonders for me, and it will for you, too!
