This is a weekly lesson plan template that can be changed to suit your needs. It is primarily for a single subject, but I have used it for two subjects in the past. I have used it in middle school and for a departmentalized fifth grade class.
It is meant to be two pages kept in a binder. You could print the pages back to back, but the margins might need tweaking. I like being able to see the whole thing, so I print each page separately.
