This writing conference form is a great way to use assessment data to inform instruction. There is a place on the form to record a baseline, scored using the Six Traits. If you need help, use my Six Traits Generic Rubric. The rest of the form has a place to evaluate every time you confer. If you're a conference "newbie", I've also included a list of question I use to get the conversation flowing when I conference. Compare this form to the Daily Five reading conferring form created by The Sisters.
