Working with small groups for reading instruction is a great way to reach every student at his/her level of instruction. Planning and organizing small groups can be a challenge, but it is worth it to see your students progress at their own pace. I developed this lesson plan template to not only plan, but also assess small group progress. There are areas to plan all aspects of your lesson and to make observations of the whole group or individual group members. This one-page teacher form is a Word document, so it is easy to modify for your needs. Keep a binder of these throughout the year and you'll have no problem monitoring your students' progress in reading.
