Here is a wonderful tool to enhance advocacy of students who are or should be receiving accommodations in the classroom. There is a place for a picture and pertinent background information on the student. PLUS, places to document accommodations that the parents are passionate about as well as accommodations in several areas such as social interaction, test taking, presentation of material, and environment. The back of the form is a guide to document meeting dates and results of those meetings. This is a great way to pass information from grade level to grade level, as well as teacher to parent.
I'm making some changes to make this form more aesthetically appearing. But, the content here is very helpful and appropriate for use with my students.
