This download contains the pages I use in my classroom to track and monitor student behavior. These sheets also come in handy at conference time or on other occasions when I communicate with parents or administration. I slide the cover into the front of a binder (the kind that has the clear plastic on the front . . . and is ready for a title page). I Then make a few copies for each student and file them in the binder (ready to use if necessary). Our focus is not on good or bad behavior, but rather on making appropriate choices and being a scholar (see the 12 Steps to Being a Successful Scholar). This is an easy, stress free way to stay organized and track student behavior.
