Use this at the beginning of the year to get to know your students. Way more fun than a regular worksheet with a list of questions. If you really want to get them working - tell them they can even use 'text message' language!
There are 4 pages here. The first one is math specific, the 2nd is for any subject, the third does not have the 12-13 year at the top right, and the fourth is blank. You can copy paste it into Word and make text boxes over it if you'd like to make your own questions.

I'm sorry, but because of the format there is no way to make it edit-able. There is a blank one that you can copy inform Microsoft word and try to create text boxes over it. Thank you!

I am putting together a community based Family Fun Math Night for preschool through High School. I think the Middle and High Schoolers would love to see something like this. I'm thinking about having them use it for a Math About Me activity. But I'm still thinking of other ways to use it. Thanks and I hope to hear from you soon.

The only way it can be edited is if you have 'Pages'. The original file was made on my Mac on that application. I can export it to Wird and email you that file, but it would take lots of reformatting to get it back to what it looks like now. I will edit the download so it has one with no questions and you'll just have to paste it in word and create text boxes. Also, I will remove the year so you may use it in the future! Thanks:)
