Student Information Sheet is an ideal introductory handout for teachers to have new students fill-out on the first day of a new course.
This document is only one-page so it can be completed quickly, but is detailed and comprehensive with 10 questions for students. Teachers get a great jump start on getting to know their students in a minimal amount of time.
This handout is general enough to be appropriate for all subjects and grade levels. There is only one reference to "math" which can be easily edited to any subject. No other editing is needed.
