This inventory is a file I shared with Scholastic when I was a mentor teacher. This is a neater, one-page version than what's posted online. I'd recommend using this with upper elementary students as it requests complete sentence answers - or at least gives room for such answers. All the lines can be a little intimidating for 3rd grade and below.
I use this form on the first day of school, and it gives me valuable information about my students. This is also a great activity for kids to complete while you're taking attendance, passing out books, and completing other necessary, first-day activities.
