This form is designed to keep track of student preparation for quizzes and tests, in order to help student, teacher, and parent analyze the effectiveness of the student's study skills and habits. On this form, students must record the times they studied, the techniques they used, and the study environment.
I created this form in order to help students in those situations when the student tells me, "I studied, but I still can't do this!" It's not a panacea, but it's amazing how often students will record a study situation that they think is perfectly OK, when it clearly is not, making the study log a useful tool for setting them straight. For example, I've had students record study times from late at night, or under "study environment," they'll write something like "in front of the TV but I wasn't paying attention to it." Right!
In the directions for the document, it states that the completed study log will earn the student bonus points on the next quiz or test, but the document is in Word format and can be easily changed if you prefer to not use such an incentive.

This is my eighth year of teaching, and I currently teach 7th grade humanities in Washington state. Curriculum design is one of my favorite parts of the job. Six years ago I redesigned our seventh grade humanities curriculum to change it from a chronological approach to a thematic approach, and I've been obsessively refining it ever since. I also started teaching some online high school courses last year, which has been a great experience for me!
