I created this form for my class secretary (a student job). They carry this form (which is designed to fit on a small clipboard) around all day long and keep track of students who are asked to take a break. They give this form to me at the end of each day and I transfer the data into an excel document (also in My Products here). The bottom of this form is for tracking how often the class gets too noisy. I have a Yacker Tracker and whenever the siren goes off, the secretary circles the next number. Thanks for looking!
