Here's a substitute form you can leave for your sub. There spaces for school, teacher name, date,and substitute name. There are two boxes for the sub to write who was misbehaving and who was helpful. There is a large box at the bottom for comments or concerns. There is space at the bottom for a sub to sign.
There is also a second blank page for the sub to write more notes.
