***Please leave feedback to let me know how you like this product.*** Students who are diagnosed with ADD/ADHD, or those who just show characteristics of these conditions, are showing up in my classroom more frequently than before. I try to communicate with parents on a daily basis to let them know how their children are doing in class with on-task behaviors. I use this classroom chart as a school-home connection. At first, I complete it with the student, but eventually the student takes over the job of monitoring his/her behavior. This Word form is easy to modify to suit your own needs.

Nice layout. I'd like to use it for my class, but I wasn't able to figure out how to modify it. I saved it into Word. I have an older version - 2007 - is that the problem?

Thank you :) Make sure you are editing the 2nd page, not the first. The 1st page was just a graphic that I put to make a decent thumbnail image. You should be able to open the 2nd page in Word and make any modifications you need. Let me know if there's any trouble with that.

Looks good as a "generic" behavior chart. I also like that you can modify to suit your own (or the student's own) needs.

This seems like it would be a great tool for your classroom. However, I'm not sure how I would be able to complete this detailed list as far as time goes every day. It's a very good idea and I know that it is usuable for others.

Thank you. Yes, it can take a bit of time to complete during the day, especially if you have more than one student to track. Depending on the age and the student, you may be able to let them self-monitor at some point.

I use a behavior chart too and have a place for parents to sign for each day. My behavior chart is briefer as I deal with Kindergarten and have to conference one on one at the end of the day. We do use a task chart which they fill in themselves as they go from center to center. It is similar to yours.
Your chart was a picture and could not be edited to suit other teachers' needs. Can you upload it as a word doc so we can modify it to meet our need?

I have been teaching for 17 years at the same school in Mandeville, Louisiana. I taught 1st grade for six years and have taught 2nd grade for eleven years. I was the director of our summer camp for three years and have worked both before and after school programs. I also worked as the web manager and technology point person for our school.
