This is a weekly behavior chart that I use in my classroom. There are specific targeted behaviors that are addressed and each day is split by morning and afternoon. The teacher comments are filled out on Fridays when the form is sent home. The parents sign and comment (optional) and return on Mondays. You can use checks, smiley faces, or pluses and minuses to notate appropriate and inappropriate behavior.

TEACHING EXPERIENCE

I have taught inclusion classes in language arts and math in the 7th grade. I have also taught self-contained 7th grade math. Currently, I teach a self-contained class of students in grades K-4, including reading, math, social studies, science, living skills, fine motor skills, and social skills.
