I try to communicate with parents on a daily basis to let them know how their children are doing in class with on-task behaviors. I use this classroom chart as a school-home connection. At first, I complete it with the student, but eventually the student takes over the job of monitoring his/her behavior. This Word form is easy to modify to suit your own needs.
