This two page document is meant to be photocopied back to back. I have copied this form on colored paper so it stands out in my students' homework folders. Students write down their weekly spelling and/or vocabulary words so they can study every night. Students have to write down what they have for homework every night. On the reverse side students record what they read on a nightly basis. On Thursday evenings students have to write a summary of what they have been reading all week. There is a space for parents to sign at the end of the week.
