This is a brilliant idea. It takes some training and time for the kids to be able to respond thoughtfully, but it is well worth it!! I feel like lessons are even more focused, and the students are more reflective about their work! Thanks!!

Would you consider revising the form to specifically include student reflections respective to Math and ELA so teachers do not have to print off two copies per student each week? Just a thought. I was wondering if you could personalize this a bit for me? I LOVE the basic form, but the bottom box where you have examples, could you change that to the word homework? I teach at a middle school, and I would love to have a space for the students to record if they are needing to complete an assignment at home. Thanks!

This product is a PDF file and in order to view/download, you need to have Adobe Reader. I tried downloading it myself to check to make sure it worked properly and it did for me.
