The form provides a spot to mark down what upper case, lower case, letter sounds, shapes, numbers 0-20 and colors a child does and does not know (by circling them for correct identification or underlining them for still working on).
It also has a check mark system to cover skills for certain areas such as math (one to one correspondence, sequencing events, etc), social studies (time and direction words, etc), science (changes in weather, etc), language arts (recognizing first name, printing letters correctly, reading books, etc), social development and play skills (how a child focuses, participates, plays with others, follows directions, cleans up, etc).
