Glad you find them useful. This is probably the most USEFUL day-to-day tool I use in my classroom. I use them with our Zinn AYPHOTUS text, primary and secondary source documents, a lot of short essays from Gilder Lehrman, documentaries etc.
My students have done close to 25 sets of these this year so far, and sometimes get tired of them - BUT then I have students coming back YEARS later telling me that, especially in AP social studies courses, they use them on a constant basis for reading text at home on their own, and even in class sometimes with other teachers!
I believe it is one of those reading comp tools (I am a big fan of SVR - Simple View of Reading Gough and Tumner - 1986 I think...) and feel that these notes hit on both the DECODING and LISTENING COMPREHENSION as I read 99.9% of covered text in my classroom while they follow along with their copies of the text (book, digital, paper class copy whatever).

I always try to include word doc / editable versions of all of my content as I realize that EVERYTHING teachers need and use will need to be edited and fine tuned for their classrooms! I still use this tool today with great effect - and even with a 1:1 paperless computer lab / classroom I hate to admit I mostly still fill this in by hand as I go back and edit / change so often along the way!

rugregory: Brilliant, thanks! I still use this with my students and have had great success with it throughout the years. Because we focus so much on non-fiction and primary / secondary sources, this is a great chance for them to be creative, yet still stay within the bounds of "historical fiction."
I have been thinking of adding other "characters" from other time periods beyond the late 19th century... Westward Expansion, etc. if there is a time period that you could use coming up in the rest of this school year let me know and maybe I can focus on that to build the next set. My pacing has been slow this year and I still have Westward Expansion, slavery leading up to the Civil War, and Reconstruction to go...

Thanks for generously sharing your work. I love anything I can use to really engage the students with informational text. Your form is well-organized, and I can tell thought went into its content and design. You may want to make a minor correction under the "During" heading. You wrote "There are no WRITE or wrong answers here!" Spell check won't pick that one up.

Yes, thank you. I am actually on here VERY rarely - though I need to get back on and update and add. The version that is on here was a very early version that has undergone several revisions... In fact, I am currently teaching in a 1:1 computer lab and I have created a PDF version that students fill out digitally and can save, print out, etc. That version has been corrected re: the write / right - but again thank you!

TEACHING EXPERIENCE
In 2012 I was chosen the Gilder Lehrman History Teacher of the Year for the state of Oregon and was awarded the American Historical Associations Beveridge Family History Teaching Prize for innovation in the teaching of history. I have previously taught all four years of High School social studies including Government and Economics, in an International Baccalaureate Small School within a large urban High School. I currently teach 8th grade Honors US History in Oregon in a 1:1 ratio computer lab predominantly using curriculum of my own design. I am also the Department Chair and a leadership/steering team member and a Data Team Leader for our school. I am also our sites EdTech Cadre representative and also teach an elective called Games, Computers, and Programming to both 7th and 8th graders. In addition, I am also an instructor in a Masters of Education credentialing program near Portland, Oregon where I teach Social Studies methods courses and Middle School Philosophy.
MY TEACHING STYLE
My teaching style is a progressive one where I try to meet my students vast array of needs. I believe in a lot of reading aloud to my students, competitive games and simulations as well as project AND problem based learning. I am a firm believer that if I can have all of my students leaving my class with competent critical thinking, analytical, comprehension and research skills - then the social studies content is secondary.
HONORS/AWARDS/SHINING TEACHER MOMENT
I have presented my methods and curriculum at numerous national and statewide conferences during the last several years including The American Educational Research Association (AERA), the National Council for the Social Studies (NCSS), and the American Middle Level Educators (AMLE) conference and the Games + Learning + Society conference (GLS) at the University of Wisconsin Madison. In 2012 I was chosen as the American Historical Association's Beveridge Family Teaching prize winner for innovation and creativity in the teaching of history and was also named the Oregon History Teacher of the Year by Gilder Lehrman, The History Channel and Preserve America.
MY OWN EDUCATIONAL HISTORY
A BA in Political Science and Public Administration with a minor in World Studies in 2000. I received my M.Ed in 2003.
