What centers did you play in today?
Dramatic play, blocks, computer, manipulatives (games, puzzles), art, reading, writing, sand, water, music, science (adjust to your centers)

What did you do in that center?
Example art – playdough, draw, paint, write, color, markers

How do you know if you can go in a center to play?
Each center has a sign up with the corresponding numbers (1-4) and pictures of the corresponding number of children. There are also smiley face stickers for each number where the children put their name tag. (Example: science, 4 children, 4 smiley faces) They can enter that center if there is a smiley face for them to put their center name tag.

What do you do before you leave a center?
Clean up and take your center name tag with you.

Who did you play with?
(Write each child’s name in here or attach a list of children. It would be good to divide them up into girls and boys)

What books did you read?
Animals, people, children, families, pets, colors, numbers, fiction, nonfiction

What was the story about in the book the teacher read to you?

What did you play with outside?
Hula hoops, balls, basketball goals, sand, water, chalk, bubbles (list whatever choices the children have)

What did your teacher talk about today? What special activities did the teacher put out for you to play with?

What was your job today?
Lunch helper, teacher helper, chair helper, table washer, blanket helper, bathroom monitor, librarian, book collector, my pal, conflict negotiator, custodian, electrician, vet. assistant, p.e. director, gardener,

What did you have for breakfast, lunch, and snack?
Handout menu to parents at the beginning of the month

I have it saved as a rich text document which usually helps. So I just included the info here. I hope you are able to use this information.
