This poem, Definition Me, is written about the way that teenagers wear masks to hind who they truly are. They hide themselves out of fear, out of the fear of rejection, loneliness, madness, and more causes. Overall, this poem takes those feelings and forms them into a poem.
Going along with "Definition Me", The Longer I Live analyzes the essence of teenagers who wear masks. Students this age tend to put on appearances because they do not like who they are. This is a poem-like, introspective, reflection.
This transition survey has been used with all the Special Education Students on my caseload for multiple years. My district has since adopted it as our transition survey form for students with an IEP ages 14 1/2 or older. This three page form is editable to best serve your needs.
