Use this sign-out sheet to log when students come and go from the classroom.

Students write their name on the log so that you can keep track of how many times the student has left the classroom, at what time they left, at what time they came back, and where they went.*************************************************************************************************************
Print a few of these pages and keep them by the door. If a student needs to leave the room to use the restroom, have them sign out, and then back in again. This is helpful to see who is abusing the privilege, and also during an emergency, you are able to see if anyone is gone.
