This document is one I use to test students' growth throughout the year. It includes 25 Early Emergent words, 25 Emergent Words, 25 Early Fluent Words and 25 Fluent Words. They are easy to color in upon mastery and give a real visual representation to students as to their increasing mastery.
To administer, I sit with a student and place a dot in the corner of all words read correctly. When the student has 5 consecutive errors I stop the assessment and tally those correct. Then students are asked to color in the words that have a dot in the corner. I keep the document in a binder to compare the results when the assessment is given again each quarter.
Once a quarter is plenty to administer this assessment.
The bottom row has a place to indicate which level a student has mastered. For example, If a student gets a score of 18, they are on the early emergent level because they scored in the first 25 words. The assessment should be given by rows, not by columns. You can place a sheet above and below the line that is being read, so that a student is not visually overwhelmed.
